1. Scope of this policy
This Privacy Policy explains how OPTIMIZE MD by Ask Health, operated by [TO CONFIRM: legal entity name] (“OPTIMIZE MD,” “we,” “us,” or “our”), handles personal information collected through this website and through our marketing and scheduling communications.
1.1 Website information and patient health information are governed differently
OPTIMIZE MD is a physician practice. Once you become a patient, the health information we create and maintain about your care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). That information is governed by our Notice of Privacy Practices [TO CONFIRM: link to Notice of Privacy Practices], which you will receive at your first visit, not by this Privacy Policy.
This Privacy Policy covers information collected through the website, such as contact form submissions, SMS consent, newsletter sign-ups, and technical data about your visit.
2. Information we collect
2.1 Information you submit to us
When you request a consultation or contact us through the website, we collect the information you enter:

- Optionally, a mobile phone number and your consent to receive text messages from us
- The program you are interested in (Foundation, Plus, Complete, or not sure yet)
- The areas you would like to improve, such as sleep, recovery, or hormones
- Any message you choose to include
Providing a mobile number and SMS consent is optional. It is never required to book a consultation or receive care. Message frequency varies, and message and data rates may apply. See our SMS Terms for the full terms.
Please do not send medical history through the website. The contact form is for scheduling. Detailed health information is collected securely at your visit.
2.2 Newsletter
If you subscribe to The Optimize Letter, we collect your email address to send it.
2.3 Information collected automatically
When you visit the website, our hosting provider automatically receives standard technical information such as your IP address, browser type, device type, pages requested, and the date and time of each request. We use it to operate, secure, and troubleshoot the website.
The website does not currently use advertising pixels or third-party advertising cookies. [TO CONFIRM: analytics tools, if any]. Before adding any analytics or advertising technology, we will update this policy. We will not place tracking technologies that disclose health information to advertising platforms.

4. How we share information

- At your direction. With anyone else only when you ask us to.
5. Text messaging and mobile information

6. Data retention
We keep contact form submissions for as long as needed to respond to you and schedule care, and then for a limited period for recordkeeping [TO CONFIRM: retention period]. If you become a patient, your medical record is kept as required by Michigan law and governed by our Notice of Privacy Practices. If you opt out of texts or email, we keep the minimum information needed to keep honoring that choice.
7. Security
We use reasonable administrative, technical, and physical safeguards to protect personal information, including encryption in transit (HTTPS) for the website. No system is perfectly secure, so we ask that you never send health information through the website form, email, or text message.
